Abstract
The outbreak of COVID-19 has fundamentally altered the delivery of healthcare services worldwide, particularly impacting patients with chronic conditions like osteoarthritis. As the pandemic surged, concerns over the heightened risk of infection led many osteoarthritis patients to postpone or forgo essential in-person medical visits. This disruption resulted in a significant reduction in hospital-based treatments, exacerbating symptoms and potentially worsening long-term outcomes for individuals with osteoarthritis. The growing reliance on telehealth during this period offers a critical opportunity to assess its effectiveness in chronic disease management, particularly in the context of Bangladesh’s healthcare system, where access to quality care is already a challenge in many regions. This study aims to evaluate the effectiveness of telehealth interventions in improving health outcomes for osteoarthritis patients during the COVID-19 pandemic in Bangladesh. Through a case study approach, this research examines how telemedicine was deployed as an alternative to traditional healthcare and investigates its impact on pain management, treatment adherence, and disease progression. The study also identifies the challenges faced by both healthcare providers and patients in utilizing telehealth, particularly in rural and underserved areas, and offers recommendations for optimizing telemedicine for future healthcare crises and routine chronic disease management. A comprehensive review of existing literature, particularly from PubMed, provides insight into the global deployment of telehealth during the pandemic. The findings highlight how telemedicine served as a critical tool in maintaining healthcare continuity for osteoarthritis patients when face-to-face consultations were restricted. Patients who utilized telehealth services reported improved management of pain and symptoms, increased accessibility to healthcare professionals, and better adherence to treatment regimens compared to those who relied on traditional in-person visits. Remote consultations, coupled with regular virtual monitoring, allowed for continuous tracking of symptoms, facilitating early interventions and minimizing the need for hospital-based care. For Bangladesh, where healthcare infrastructure often struggles to meet the demands of chronic disease management, telehealth has proven particularly beneficial. Rural areas, which frequently face a shortage of healthcare providers, were able to leverage telemedicine to bridge the gap between patients and specialists. The accessibility and convenience of telehealth services not only minimized disease progression in many patients but also enhanced overall patient satisfaction. The study indicates that telemedicine interventions, when integrated with existing healthcare systems, can reduce the strain on physical healthcare facilities while maintaining a high standard of care for chronic disease patients. However, several challenges were identified in the deployment of telemedicine during the pandemic. Limited access to the internet, lack of digital literacy, and the need for physical examinations were major barriers, particularly in rural Bangladesh. In addition, some patients and healthcare providers expressed concerns about the adequacy of telehealth for diagnosing and managing complex cases, which typically require hands-on assessments. There were also issues related to the technological infrastructure necessary to support large-scale telehealth services, particularly in areas with poor network connectivity. Despite these challenges, the findings of this study underscore the crucial role of telehealth in ensuring continuity of care for osteoarthritis patients during the COVID-19 pandemic. The success of telemedicine in managing chronic disease in such a crisis demonstrates its potential for long-term integration into Bangladesh’s healthcare system. Telehealth offers a viable solution for managing chronic diseases, especially in remote areas where access to specialist care remains limited. The study concludes by recommending that healthcare systems, particularly in developing countries like Bangladesh, invest in expanding telehealth infrastructure, improving internet accessibility, and enhancing digital literacy among both healthcare providers and patients. These steps are essential to optimizing telemedicine for chronic disease management and ensuring better health outcomes in future public health crises. Additionally, policymakers should focus on integrating telehealth services into standard healthcare practices to improve the long-term management of chronic diseases, such as osteoarthritis, beyond the pandemic.
