Assessment of the Effectiveness of Community-Based Health Education Programs on Hypertension Awareness and Control among Rural Adults in Bangladesh

Abstract

Hypertension remains a growing public health concern in Bangladesh, especially in rural areas where awareness, prevention, and control mechanisms are still limited. This study aimed to evaluate the effectiveness of a structured community-based health education program in improving awareness and control of hypertension among rural adults in Bangladesh. A quasi-experimental study design was adopted, involving 400 participants aged 30 years and above from two rural communities. One community received a structured six-month health education intervention, while the other acted as a control group. The intervention included interactive sessions covering topics such as the definition and risks of hypertension, dietary modifications, the importance of regular physical activity, salt reduction, and adherence to medication. These sessions were delivered by trained community health workers, ensuring culturally appropriate messaging and accessible content. Pre- and post-intervention assessments were conducted using a standardized questionnaire and direct blood pressure measurements. The results revealed a statistically significant improvement in awareness and behavioral outcomes in the intervention group compared to the control group (P<0.01). Knowledge scores improved by 45%, and the proportion of participants with controlled blood pressure increased from 32% to 58%. Additionally, participants reported lifestyle improvements such as decreased salt intake, enhanced physical activity, and improved medication adherence. The findings indicate that community-based health education is an effective strategy to raise awareness, influence behavior change, and improve hypertension control among rural populations. The involvement of local health workers and the use of context-specific education materials contributed to the program’s success. These results underscore the value of incorporating community health education initiatives into the broader national non-communicable disease (NCD) control strategy. This study provides a practical framework for scalable and sustainable interventions in low-resource settings. It highlights the urgent need to invest in community-level education as a tool to combat chronic diseases such as hypertension and reduce the risk of related complications in underserved populations across Bangladesh.
